Community Living Group Homes

Community Living-Group Home Supports Services provide individuals with intellectual disabilities, with development and maintenance of skills related to activities of daily living, instrumental activities of daily living, and socialization, through application of formal teaching methods in a community residential setting.

  • Transportation to community outings
  • Supports for community engagement and activities.
  • Medical Appointments
  • 24/7 Nursing Support Services
  • 24/7 hours staffing and support services.
  • Provision of Assistance with Activity of daily living.
  • Assistance with socialization and learning socially acceptable behavior.

Supported Living Services

Supported Living Support Services enable individuals to live in a house or apartment of his or her choice located where he or she wants to live. Services include:

  • Provision of assistance with Learning self-direction and problem-solving related to performing activities of daily living.
  • Provision of assistance to individuals to engage in community-based activities of choice.
  • Provision of assistance and facilitation with finding an apartment or home, roommates based on individual preferences and choice.
  • Provision of coordination, training, support, and/or supervision as indicated in the person-centered plan.
  • Services may include up to 24 hours of support per day as specified in the Person-Centered Plan

Community Development Services (CDS)

(CDS) aids the participant to develop and maintain skills related to community membership through engagement in community-based activities with people without disabilities. Services include:

  • Provision of activities which facilitate and promote integration and
  • inclusion in the participant’s chosen community
  • identification of a path to employment for a working age participant
  • Travel training
  • Self-advocacy classes and activities
  • Local community events
  • Volunteering within a non-profit organization
  • Performing a paid or unpaid internship
  • CDS also include transportation to, from, and within activities.
